*A serious complaint is defined as a report of a serious adverse event that significantly compromises clinical outcomes, such as poor image quality, or a failure to communicate results.Complaint Mechanism
1. Immediately on receipt of a consumer serious complaint the consumer will fill out a consumer complaint form with the assistance of a mammographer.
2. Most consumer complaints can effectively be addressed by the facility, (refer to policy). After completion of the consumer complaint form the complaint will be addressed by mammographer and the Radiologist (if clinical in nature), then by the Department manager and if still unresolved, finally by the Risk Manager.
3. If the problem is not resolved at any facility level, then the consumer will be provided adequate directions to report this complaint to the accreditation body.
