Find an existing “evaluation” form from a program/ workshop/session you have attended or presented in your workplace and critique it in terms of whether or not it fulfills the major points as presented in the Study Notes and your reading assignments so far. Include in your response:

· The major points you are using in your critique
· The purpose you are using this evaluation form
· What you would do differently and why
· Identify the program’s objectives, over-all goal(s) of the training, whether they were met or not, etc. (Be sure to include a copy of the “evaluation form” you are using.) (2-3 pages)
