Overview
A 3000 word written assignment based on a patient from your clinical / student nursing experience / placement. The selected patient must have a long-term condition that is relevant to the themes of the module.
This written piece must demonstrate achievement of all intended learning outcomes outlined below.
1. Demonstrate an understanding of the pathophysiology of long-term conditions and the clinical manifestation in response to this.
2. Support people experiencing multiple health care problems and plan their nursing care as part of a multidisciplinary team.
3. Reflect on the medical and nursing management of long-term conditions in practice in relation to recommended strategies, guidelines and policies.
4. Review the nursing function in supporting and promoting self-caring strategies for patients and their families.
Proposed Structure
A proposed structure for the written assessment is provided below.

A clear introduction must be provided, in which the reader is introduced to the purpose and focus of this written piece of work.

A written paragraph which provides a background history to the selected patient with explicit identification of the related long-term condition.
? Pathophysiology and related clinical manifestations – 500 words
Provide an overview of the underlying pathophysiology of your selected client’s condition and any relevant clinical manifestation. In order to demonstrate knowledge and understanding, this discussion must be supported by a sustained use of literature and be clearly related to the selected client.
? Context of nursing assessment – 200 words
For this section, a clear and concise overview of the importance of a systematic approach to nursing care delivery is required. Additionally, some related discussion on the selected model of nursing care would be beneficial.
? Formulation of care plan – 500 words.
For this section, you are required to formulate a nursing care plan using the template provided. The care plan must be based on Orem’s model of nursing. When formulating the care plan, please select only ONE element from Universal self-care requisites and ONE element from Developmental self-care requisites OR Health-care deviation self-care requisites.
NB: Please do not attempt to complete all elements of the care plan.
? Rationale for care delivery – 1300 words.

o Nursing actions/evaluative strategies;
o Related multi-disciplinary involvement;
o Proposed method of care.

Assignment presentation
This written assessment must be word-processed, utilising Microsoft Word and presented in Arial font size 11pt. Double-line spacing must be applied throughout the main text of this piece of work. Pages should be numbered and each page of the assignment must have your university ID number in the header of each page. The main text of the assignment must be aligned to the left of the page. An accurate and consistent referencing approach must be undertaken in accordance with guidelines outlined in the current referencing guide.
Word Count
The indicative word count set for this piece of work is 3000 words +10%. Your programme handbook outlines details of penalties relating to producing pieces of work in excess of the word count. Please note that is unlikely that you will meet all the intended outcomes in sufficient detail where an assignment is short of the indicative word count.
Marking Criteria
