: You will include the following information about the participant: age, grade, gender any information regarding health, educational / learning needs, academic, social or behavioral strengths and weaknesses as related to the challenging behavior.**do not relate stories about the student or give anecdotal stories regarding the student or her/his family.

Challenging Behavior (Operationally Defined)

The Challenging Behavior is defined in terms that are observable, measurable, and clearly understood. A rationale based on benefit to the student is provided as to why this is a priority behavior that needs to be addressed.

 

Replacement Behavior (Operationally Defined) The Replacement Behavior is defined in terms that are observable, measurable, and clearly understood. A rationale will be provided as to how the proposed change in behavior will help the participant. Data Collection: Sufficient

Relationship Statement

A relationship statement about the function of the behavior has been written. The relationship statement identifies specific antecedents that occasion the behavior as well as the most common consequence. The rationale behind the relationship statement is described with references to specific data to support the determination. The function has been correctly defined. (This is not a statement about your relationship to the participant. It is about the relationship between the behavior and the function. You must not only state the function but what the antecedents and consequences surrounding the challenging behaviors are and why / how they led you to determine the function).
