Task description
This assessment item ? written assignment part A will form the first stage of an evidence – based client education strategy related to a chosen chronic condition. Students will present an evidence-based education plan in essay format that is related to one of the clients presented below. Students will demonstrate their ability to gather evidence from a number of appropriate sources (recent and appropriate literature) about the specified chronic condition and to access information on client education, specifically related to the client they have chosen.

A 36 year old female diagnosed with rheumatoid arthritis
Part A: Evidence-Based Education Plan (refer to Essay Marking Criteria below)

? The education plan should address the following areas:
o Introduction

o Education strategies ? Description of evidence?based educational strategies and a rationale for their use for the client you have chosen and their extended family/community e.g. print media, one-to-one teaching, audio visual media, computer assisted learning etc .
o Conclusion/Recommendations
Word Limit: The Education Plan will be a maximum of 1500 words. References need to be supplied but the reference list is additional to the 1500 words.
Referencing Style: APA 6th. Minimum 15 references less then 5 yrs old

o current evidence-based treatments
o client self-management issues
Education strategies
 Clearly articulates the challenges for this client
 Identifies the particular focus of education strategies
(lifestyle changes, self- management strategies etc)
 Identifies the appropriate type of educational strategies
(print media, one-to-one teaching, audio-visual media,
computer assisted learning etc
 Clearly focuses the education plan on extended
family/community.
Conclusion/Recommendations
 Provides a conclusion and/or recommendations at the end
of the plan.
Research
 Provides evidence to support all points/claims made in the
plan
