Design a questionnaire to address a research question in the topic area of your choice. The questionnaire will contain at least 10 questions and not more than 20 questions. The questionnaire should be between 1 to 3 pages in length. Accompanying the questionnaire will be a rationale (no greater than 800 words), which justifies the design and structure of the questionnaire.
Tasks for Questionnaire
Construct a series of questions that will allow you to address your research question.
Ensure that your questions include:
o At least 3 demographic questions, such as age, gender, income, education etc. You may choose more than 3 if necessary.
o Consider including different types of survey questions: open-ended; multiple choice; ordinal scale; interval scale (a likert scale question is often useful); ratio scale
Make sure your question wording is clear and unambiguous. Try answering the questions yourself. Does the question make sense? Are all possible response options covered?
Provide instructions for respondents as necessary, for example tick one box only or if No, go to Question 2
Take care with formatting. Refer to readings for help.
Tasks for Questionnaire Rationale
The Rationale will explain the topic area chosen and the research question being addressed.
A short statement should also be provided for each question. The statement will explain what the question is measuring, how it contributes to overall goal of the questionnaire and any other important information (like use of a particular format). Allow a few sentences for each question.
