Choose a job you have now or have had in the past.

Choose one flexible work arrangement you would like to establish for yourself.

Familiarize yourself with the different types of flexible work arrangements:
 eBook page 109 and 110

4.     
 
Ensure your proposal does not exceed two pages.
 

5.     
 
If a question/item does not apply to you, explain why in the answer space.
 
 
Do not just leave it blank.
 
 
For example, if you are a one-person company without even a secretary, you could write in the Effect on other employees section”, This section is not applicable to me because I am the entire company and there are no other employees”.

When you fill out a section, remove all the pre-existing text from when you downloaded it. 
 

7.     
 
In the first answer box Proposal” be sure to specify which flexible work arrangement you are applying for and bold the name of it (e.g. telecommuting).

8. 
 
You can type your name at the end of the proposal instead of signing it.
