Back in 1996 an article was published that contented Blue Cross had taken some flak for charging more for patients who sought specialists out side of the Blue Cross network (Shinkman, 1996). While this was controversial in the mid 90s, it is not really an issue today. What do you think has changed in almost 10 years that this is no longer considered controversial.
