Please open each of the documents in this folder. Remember you are only required to submit the demographic page and the case presentation. Please do not submit the screening/intake/assessment form or the Global Criteria worksheet. These documents along with the case presentation sample are simply provided to assist you in completing the case presentation. The intent is to remind you of the things you learned about what goes into an assessment through the assessment forms and also to provide the Global Criteria in a single document so that you don’t have to review all units to determine if you are including examples of counselling skills that are consistent with the Global Criteria. If you do not use them that is fine. All that is required is a case presentation that is a minimum of 6 pages double spaced. The comprehensive sample provided is more detailed and longer in order to provide you with lots of examples–it is not as an example of what is expected of you.
