1. Identify the health need that you will tell the Springdale Community Health Foundation that you want to address in your community.2. Explain your rationale for saying your area needs a health education program on the issue you identified. This should include a discussion of the geographic distribution of the issue, using appropriate sources of health-related data. Provide statistics that demonstrate the need for the health education program that you are proposing. Specifically, what is the incidence of the disease or condition in the geographic area or population you will be considering? How does this compare to the rest of the state/country?3. Briefly describe how this disease, condition, or health risk is caused. Specifically mention biological, social, environmental, and behavioral factors. Also, discuss predisposing, reinforcing, and enabling factors specific to this health issue. State which factors are predisposing, which are reinforcing, etc.
