The overall goal of the Session Long Project is to examine health care delivery in the United States from a personal perspective. Please limit your paper to 2-3 pages.

Assignment expectations for module 1:

1. Explain the type of health care coverage you currently have. For full credit, refer to your insurance handbook or Web site. Do not just write what you know about your policy.

Name of plan
Source – (Personal, employer, government, parents…)
Your out-of-pocket costs: co-payments and deductibles
Percentage of coverage (100%, 80/20, 70/30)
Major limitations ? Are there limitations to the plan such as use of out of plan providers or pre-existing conditions? (Refer to your insurance handbook or Web site and include details.)
Exclusions – What procedures or treatments are not covered? (Refer to your insurance handbook or Web site and include details.)
