Now we will analyze the data and plan for a population-specific project.Secondary Analysis of Already Existing DocumentsAfter collected data have been analyzed, it may be helpful to go back and review the documents that were collected during the data-gathering phase or to find additional data that have already been collected by another source.Data Interpretation (Diagnosis)After the data have been collected and thoroughly analyzed, it is important to interpret the information to identify any significant themes or trends. This can be done by organizing the information that has been collected during data gathering and data generation. You are looking for themes or indicators of an existing or potential health concern or issue. What factors are causing your population to become unhealthy, ill or causing death. This analysis should result in discovery of community health problems (needs) and community health strengths (resources) of the population.Discuss aspects of the health concern not being addressed despite the efforts of the partners involved.Describe the ultimate outcome(s) or goal(s) for improvement related to the health concern.Answering the following questions may assist in data interpretation:
What similarities are apparent between the data that were gathered and the data that were generated?

What are the weakness and strengths of this community?
In what areas is improvement needed in this community?Select a Primary Prevention TopicFrom the top health concerns for your community you will select a health promotion or primary prevention topic that will prevent the issue from causing illness in the first place. For example: the Promotion of Immunizations can reduce, eliminate, or maintain elimination of cases of vaccine-preventable diseases. Another example: the Prevention of Obesity can reduce ones risk factors for development of diseases such as diabetes and heart disease. Your will need to reference Healthy People 2020 when selecting your topic and state which objective you are basing your project on. Recommend nursing actions/interventions to improve the health concern and achieve your stated goals and objectives.
