For a diabetic Clinic or Centre in a hospital , select a set of ten indicators ( 2
structure, 4 process and 4 outcome) clarifying the following;
1.
The rational for the selection of those indicators
2.
Specify the literature references for the indicators mentioned in your set
3. Define each of the selected indicators with respect to type (structure, process
or outcome), ( numerator or denominator) and all inclusions and exclsions
4. Define Data collection plan in terms of
i- Collection frequency ii- Data Sources
iii- Method?

5. Is there a target or goal for this indicator? If yes, what is target or goal for
this indicator i.e. number, rate, percentage or volume.

Note: indicators should be comprehensive and of a high volume problem and a
high risk or problem prone condition.
