(incident choose by me: during my placement in the emergency department, I realise due to the fast turnover rate everything is rushed, nurses and doctors try to discharge patients or transfer them to a ward asap, best within 4 hours. therefore, I rarely see nurses do 5 rights check and 3 medication checks before administration medications. this may result in wrong medication given or wrong dose given. please elaborate on this.)
